The Patient Health Questionnaire-9 (PHQ-9) is a validated self-report screening instrument for depression, developed by Drs. Robert L. Spitzer, Janet B.W. Williams, Kurt Kroenke, and colleagues. Its nine items correspond to the diagnostic criteria for major depressive disorder in the DSM-IV. It is one of the most widely used depression screening tools in primary care worldwide and has been released by Pfizer for unrestricted use.
Each of the nine items is scored 0 to 3 (Not at all = 0, Nearly every day = 3), for a total score of 0 to 27:
